  • A current list of officers and directors, including contact information
  • Signatures of two officers - the President and the CFO/Treasurer
  • IRS Form 990
  • Audited financial statement (if revenue is between $250,000 and $750,000)
  • Audited financial statement using cash basis measurement only (if revenue is greater than $750,000)
